To assure physical fitness for the rigors of athletic competition and proper authorization to expedite medical assistance in the case of an
emergency, all student athletes must provide the College the information requested on the three forms listed below. Completed forms are to be submitted to the
athletic trainer each school year and are due prior to the studentís first practice session.

Insurance / Medical Travel Form (All athletes)
Preparticipation Medical History Evaluation and Athletic Physical Form (All athletes)

Playing soccer in college is such a gift that God has given me. I love playing soccer. Even before Christ saved me, soccer was my
escape when things were rough in my life. It was everything to me. But now that God has so gloriously captivated my life, He's letting
me use my love for soccer to help me love Him more. I always think ahead to the future, and I just know that some of my best college
memories will be from playing. God has also taught me so much through playing. Probably the biggest thing being, to just step back and
realize that every game could be of eternal significance, if I would just play it that way. To play for Him. Lessons, life long
friendships, and good times.
