Participation Forms


To assure physical fitness for the rigors of athletic competition and proper authorization to expedite medical assistance in the case of an emergency, all student athletes must provide the College the information requested on the three forms listed below. Completed forms are to be submitted to the athletic trainer each school year and are due prior to the student’s first practice session.

Information Release Authorization Form (All athletes)

Insurance / Medical Travel Form (All athletes)

Preparticipation Medical History Evaluation and Athletic Physical Form (All athletes)

Cougar Spotlight



Playing basketball for CCC means a lot to me. To me, being a Christian athlete means to play a sport all to the glory of God and not to self, always up lifting Christ and keeping your testimony on and off the court. No matter if I am playing a game or just practicing, I can't let my flesh get the best of me through athleticism and rivalry.
